Referrals
All New Patients must be referred to our practice as we are a referral-based clinic. All referrals are reviewed prior to scheduling by our physicians.

- Diagnosis from referring Healthcare Practitioner /Medical Records
- Current Imaging (MRI or CT scan preformed within the last year)
- Previous treatments or therapies
