The Community Health Assessment is a health opinion survey that gathers information about the health and well-being of Bladen, Cumberland, Harnett, and Hoke County residents and helps identify ways to address those needs and concerns. The survey is being conducted by local Health Departments, Cape Fear Valley Health and other agencies.
